Referral Guidelines

Who can make a referral?

We accept referrals from

  • GPs
  • Hospitals
  • Voluntary Agencies
  • Mental Health Teams
  • Housing departments

N.B. There is a daily charge for anyone referred to a Hackney-based centre. There is also a charge for anyone referred from another borough to a Haringey or Islington-based centre. Such charges are payable by the referring borough.

Additional information

Before we can process the client's application please ensure that all sections of the referral form are completed correctly.

It is important that we receive a medical or social workers' report and a recent CPA review where necessary. Failure to include these on request or with the referral form will cause delay to the application.

What happens next?

When we receive the referral the details will be discussed with the Day Centre manager and if the client fulfils the eligibility criteria we will write to the client to arrange an informal visit, with a copy of the letter sent to the referral body.

If the client is suitable s/he will be offered a month's trial period at the centre after which they will acquire full membership.
